North Halifax PCN is a well-established network of four collaborative practices delivering outstanding care to our population for over five years. Our vision is to build services that improve patient outcomes, share clinical skills consistently, ease practice workloads, and develop premises fit for future growth.

We are seeking an Advanced Practitioner (Nurse, Pharmacist, Paramedic, or other AHP) to join our innovative team. This role offers a fantastic opportunity to work across multiple practices, develop advanced clinical skills, and support enhanced services within the PCN.

The role will support any future PCN Enhanced Services projects or workstreams as directed by the PCN Manager and the PCN Board representatives. Clinical supervision will be provided by the Clinical Director as well as regular individual Practice sessions. You may be asked to support junior colleagues with clinical supervision and development.

North Halifax PCN is a well-established, forward thinking collaboration of four GP practices (Beechwood Medical Centre, Caritas Group Practice, Keighley Road Surgery, and Plane Trees Group Practice) serving approximately 38,000 patients across North Halifax. We operate a hub and spoke model that supports integrated, patient centred care.

Our multidisciplinary team includes Mental Health Practitioners, Clinical Pharmacists, First Contact Physiotherapists, Podiatrists, Paramedics, Ageing Well Practitioner, and Phlebotomists. This diverse clinical mix allows us to address a wide range of health needs and deliver high quality, personalised care.

Main duties of the job
  • Conduct full clinical assessments, diagnosis, and autonomous prescribing.
  • Manage acute and chronic conditions using evidence based practice.
  • Provide holistic, patient centered care considering physiological and psychological needs.
  • Support advance care planning and admission avoidance strategies.
  • Lead and contribute to clinical education, supervision, and team development.
  • Work collaboratively across the PCN and wider healthcare system.
  • Prioritise and manage complex caseloads including frailty and long term conditions.
  • Participate in audit, service improvement, and clinical governance activities.
  • Maintain accurate and contemporaneous patient records.
Person Specification Experience
  • Knowledge of clinical governance framework.
  • Ability to write and present information effectively and concisely.
  • Broad knowledge of professional, educational and other health care policy.
  • Knowledge/experience of development advanced clinical skills/roles.
  • Knowledge of SystmOne & EMIS.
Qualifications
  • Registered with NMC or HCPC with advanced clinical practice/Advanced Nurse Practitioner qualification.
  • Experience in nurse led/HCPC management of minor illness, or minor ailments and injuries.
  • Evidence of post registration study at undergraduate level (Level 6 or 7 in minor illness).
  • Undertake full clinical assessments and prescribe autonomously.
  • Manage complex frailty cases and long term conditions.
  • Experience of clinical and educational programmes.
  • Relevant Education Qualification (PG Cert Clinical Education or equivalent).
  • Master degree in advanced clinical practice.
  • Provide leadership and support for enhanced services and PCN development.
  • Experience of participating in care planning, audit, and continuous service improvement.
