Role: Advanced Clinical Practitioner

Location:Arrowe Park UTC, Wirral

Hours:8am-6.30pm Mon-Fri (1-3 days)

Contract: Permanent

Main duties of the job
  • See and assess patients presenting with urgent but non-life-threatening conditions (minor injuries, infections, acute illness, and exacerbations of long-term conditions)
  • Take comprehensive histories, conduct examinations, and formulate differential diagnoses
  • Use clinical judgement to manage risk and prioritise patients effectively
  • Provide timely and evidence-based treatment plans
  • Prescribe medications and interventions where appropriate
  • Arrange onward referral if needed
  • Communicate clearly with patients, carers, and multidisciplinary colleagues
  • Record consultations accurately and efficiently using EMIS &SystmOne
  • Ensure all documentation meets governance and medico-legal standards
  • Provide guidance and support to the clinical team as needed (e.g. nurses, or trainees)
  • Participate in clinical discussions, learning sessions and audits
  • Recognise and respond to safeguarding concerns in adults and children
  • Escalate concerns appropriately and follow local safeguarding protocols
  • Follow protocols and contribute to audit, feedback, and service development
  • Maintain registration and revalidation through CPD and professional development

Job responsibilities The following are the core responsibilitiesof the Advanced Clinical Practitioner.

This list is intended to summarise the keyresponsibilities and is not intended to cover every task that may be requiredfor the role. There may be, onoccasion, a requirement to carry out other tasks. This will be dependent uponfactors such as workload and colleague levels:

Clinical Knowledge and Skills

Assess and treat a wide range of patients, withundifferentiated, diagnosed and undiagnosed conditions, in a variety ofsettings and the patients own home, both face to face and by telephoneconsultation.

Undertake direct clinical management of patients,including history taking, systemic physical assessment, diagnosis,formulation of clinical management plan and prescribing appropriatetreatment, in accordance with both national and organisational guidelines.

Manage complex and changing clinical situations includingdeteriorating patients, palliative and end of life patients and mental healthpresentations.

Prescribe and supply or administer medication andtreatments according to own scope of clinical practice and organisationalpolicies.

Participate in the OOH triage service if required.Accurately assess, prioritise and arrange review or referral of patients, ifappropriate, using a variety of techniques to elicit and interpret history ofan event or illness, including past medical history and drug history, inorder to reach a clinical decision. Offer safe and evidence-based advice forself-care if appropriate.

To refer to and liaise with other agencies, includingsecondary care providers, community nursing teams, hospice colleagues asrequired.

Use excellent communications skills to ensure patientsand their loved ones or carers gain the fullest understanding of their healthneeds and services available, including those who may be at risk because ofbarriers to communication.

Maintain accurate and contemporaneous records.

Act as an advocate for the patient.

Actas a role model for colleagues, providing best practice in all aspects ofcare, and ensure care is guided by agreed policies and procedures. Thisspecifically includes practice relating to safeguarding and medicinemanagement.

Towork as part of a wider team to ensure that the care of people living withFrailty is based on relevant best practice guidance including, NSFs and NICEguidance and ICB policies.

Responsibility for PatientCare

Recogniseand work within own competence and professional code of conduct as regulatedby the HCPC.

Produceaccurate, contemporaneous and complete records of patient consultation,consistent with legislation, policies and procedures.

Prioritise,organise and manage own workload in a manner that maintains and promotesquality.

Delivercare according to NSF, NICE guidelines and evidence-based care.

Participatein the maintenance of quality governance systems and processes across theorganisation and its activities.

Inpartnership with other clinical teams, collaborate on improving the qualityof health care responding to local and national policies and initiatives asappropriate.

ProactivePatient Care Specific (where required):

Assess,diagnose, plan, implement and evaluate treatment/interventions and care forpatients.

Clinicallyexamine and assess patient needs from a physiological and psychologicalperspective, and develop person centered and holistic plans of clinical careaccordingly.

Assess,diagnosis, plan, implement and evaluate interventions/treatments for patientswith complex needs seeking advice as appropriate.

Prescribeand review medication within personal ability for therapeutic effectiveness,appropriate to patient needs and in accordance with evidence-based practiceand national and practice protocols, and within scope of practice.

Workwith patients in order to support compliance with and adherence to prescribedtreatments.

Provideinformation and advice on prescribed or over-the-counter medication onmedication regimens, side-effects and interactions.

Prioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergencycare and working with our multiple stakeholders to ensure best patient care,ensuring patients receive the right care at the right time.

Supportpatients and their families to adopt health promotion strategies that promotehealthy lifestyles, and apply principles of self-care.

Assess,identify and refer patients presenting with mental health needs in accordancewith the NSF for Mental Health.

Communication

Communicateeffectively with patients and carers, recognising the need for alternativemethods of communication to overcome different levels of understanding,cultural background and preferred ways of communicating.

Anticipatebarriers to communication and take action to improve communication.

Utiliseand demonstrate sensitive communication styles, to ensure patients and theirfamilies are fully informed and consent to treatment.

Maintaineffective communication within the practice environment and with externalstakeholders.

Actas an advocate for patients and colleagues.

Act as a representative of the organisation to ensure OneWirral CIC is held in high regard

Maintain up to dateprofessional registration.

Maintain clinical credibility by demonstrating continuousprofessional development.

Maintain professional training and registrationrequirements for area of responsibility.

Undertake training and education as required and ensurethe highest standard of evidence based practice.

Analytical Tasks

Usetechnology and appropriate software as an aid to management in planning,implementation and monitoring of care, presenting and communicatinginformation.

Reviewand process data using accurate SNOMED codes in order to ensure easy andaccurate information retrieval for monitoring and audit processes.

Understandresponsibility of self and others to the practice and primary care trustregarding the Freedom of Information Act.

Presentclinical data and information to the MDT team to agree patient pathway.
