Job Description
Kings Medical Practice has a great opportunity for an Advanced Clinical Practitioner to join our forward -thinking training practice, in Normanton, justoff the M62 corridor.

Our large modern building was personally designed with theintention of hosting all services at one site, for the benefit of patients andour staff and includes 24 fully air- conditioned consulting rooms, an operatingtheatre, education suite along with ultrasound, dermatology, neurosurgery,podiatry, community nursing and an NHS dentist.

We are focused on promoting work-life balance for our clinical team with a 90% reduction in administrative burden due to innovative roledevelopment and unique technological investment.

Our extensive pharmacy team lead on all medication activityincluding acute and repeat prescribing, reconciliation of medication, SMRs,promoting eRD and all quality initiatives.

We have invested in our acute and emergency response serviceand now have a dedicated multidisciplinary team of GPs, ACP's, Specialist Nurses and PAs.

The culture of training is a core identity of the Practiceand manifests across the MDT. Development is actively encouraged with a strongethos of growing our own. Full financial and practical support is provided todevelop special interests, leadership activities.

Main duties of the job The post holder is responsible for ensuring the delivery of safe and effective nursing care to the whole practice population. As an Advanced Clinical Practitioner (ACP) the post holder is an experienced practitioner, who acting within their professional boundaries, will provide evidenced based care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing clinical leadership and direction for the practice team as required.

We are looking to appoint an Advanced Clinical Practitioner for 25.5 hours per week, there is minimal administration and we have low visit rates.

NOTE: Interviews will be held on Monday 13 October.

Job responsibilities Clinical Practice:

Provide direct access to an ACP for thegeneral practice population through clinical triage

Providesafe, evidence-based, cost-effective, individualised patient care throughthe assessment, diagnosis, implementation and evaluation oftreatment/interventions and care for patients presenting with anundifferentiated diagnosis

Clinically examine and assess patient needsfrom a physiological and psychological perspective, and plan clinical careaccordingly

Assess, diagnoses, plan, implement andevaluate interventions/treatments for patients with complex needs

Refer patients directly to otherservices/agencies as appropriate

Proactively identify, diagnose and managetreatment plans for patients at risk of developing a long-term condition (asappropriate)

Diagnose and manage both acute and chronicconditions, with a potential for a focus on learning disabilities and or frailty,integrating both drug- and non-drug-based treatment methods into a managementplan

Prescribesafe, effective and appropriate medication as defined by currentlegislative framework and review medication for therapeuticeffectiveness, appropriate to patient need, evidence-based practice andnational and practice protocols, and within scope of practice

Work with patients in order to supportcompliance with and adherence to prescribed treatments

Provide information and advice on prescribedor over-the-counter medication on medication regimens, side-effects andinteractions

Prioritise health problems and interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care

Support patients to adopt health promotionstrategies that promote healthy lifestyles, and apply principles of self-care

Contribute tothe practice achieving its quality targets to sustain the high standardsof patient care and service delivery

Work withmutli-disciplinary team within the Practice, and across the wider area topromote integrated and seamless pathways of care

Demonstratecritical reflection on own practice and openness to change

Maintain their professionalregistration

Recognise and work within owncompetence and professional code of conduct

Produce accurate, contemporaneous andcomplete records of patient consultation, consistent with legislation, policiesand procedures

Prioritise, organise and manage ownworkload in a manner that maintains and promotes quality

Promoteevidence-based practice through the use of the latest research-based guidelinesand the development of practice-based research.

Assess and monitorthe effectiveness of their own clinical practice through the quality assurancestrategies such as the use of audit and peer review, bench marking and formalevaluation

Participate incontinuing professional development opportunities to ensure that up-to-dateevidence-based knowledge and competence in all aspects of the role ismaintained

Initiate and participate in themaintenance of quality governance systems and processes across the organisationand its activities

Utilise the audit cycle as a means ofevaluating the quality of the work of self and the team, implementingimprovements where required

In partnership with other clinicalteams, collaborate on improving the quality of health care responding to localand national policies and initiatives as appropriate

Evaluate patients response to healthcare provision and the effectiveness of care

Support and participate in sharedlearning across the practice and wider organisation

Use a structured framework to manage,review and identify learning from patient complaints, clinical incidents andnear-miss events

Assess the impact of policyimplementation on care delivery

Monitor and develop the performance ofthe nursing team providing feedback as appropriate

Understand and apply legal issues thatsupport the identification of vulnerable and abused children and adults, and beaware of statutory child/vulnerable patients health procedures and localguidance

Management and Leadership

To support structures are in place forthe smooth running of the practice

Encourage others to make realisticself-assessment of their application of knowledge and skills, challenging anycomplacency or actions that are not in the interest of the public and/or usersof services

Work with practice management to ensuresufficient staff of appropriate ability, quality and skill-mix are available tomeet current and future service delivery, that selection and recruitmentprocesses are effective.

Contribute to the planning andimplementation of teaching and support for staff development in order tomaximise staff potential and actively promote the workplace as a learningenvironment, encouraging everyone to learn from each other and from externalgood practice

Critically evaluate and reviewinnovations and developments that are relevant to the area of work

Act as an advisorto other members of the Primary Healthcare team.

Encourage anddevelop teamwork within the practice

Participate inmulti-disciplinary protocol development

Identifyappropriate opportunities to delegate both clinical and administrative tasks tomore junior staff.

Help the practiceoperate in a cost-effective manner.

Participate inpractice meetings and practice management meetings, reporting progress asrequired.

Participate inaudits and inspections as appropriate.

Work closely withthe doctors and administrative managers in the setting up and/or improving ofpractice systems for monitoring/measuring performance against ClinicalGovernance and Quality Indicator targets.

Take a lead role in planning andimplementing changes within the area of care and responsibility

Contribute to the development of localguidelines, protocols and standards

Maintain effective communication withthose responsible for the overall commissioning and procurement process
