Job Description
Job Summary:
We are seeking a highly skilled and motivated Advanced Clinical Practitioner to join our Urgent Community Response Team in Stoke. The successful candidate will play a crucial role in delivering high-quality urgent care services within the community. As a key member of the team, you will be responsible for making senior clinical decisions, working autonomously, and contributing to the effective functioning of the Urgent Community Response service.
Key Responsibilities:
- Senior Clinical Decision Maker:
- Utilize advanced clinical skills to assess, diagnose, and treat patients with urgent care needs.
- Make sound clinical decisions, providing leadership and expertise to the Urgent Community Response Team.
- Collaborate with other healthcare professionals to ensure seamless patient care.
- Autonomous Working:
- Work independently and autonomously within the scope of advanced clinical practice.
- Take the lead in managing complex cases, providing timely and effective interventions.
- Urgent Community Response:
- Respond promptly to urgent care requests within the community.
- Conduct home visits and assessments, delivering patient-centered care tailored to individual needs.
- V300 Prescriber:
- Hold a V300 prescribing qualification, demonstrating the ability to prescribe medications within the advanced practitioner role.
- Prescribe and review medications in line with professional standards and guidelines.
- Car Driver:
- Possess a valid driver's license and have access to a car for transportation.
- Be willing and able to travel to various locations within Stoke to provide timely responses to urgent care needs.
Qualifications and Experience:
- Registered as an Advanced Clinical Practitioner with a relevant professional body.
- Hold a V300 prescribing qualification.
- Proven experience as a Senior Clinical Decision Maker within urgent care settings.
- Ability to work independently and autonomously.
- Excellent communication and interpersonal skills.
