Job Description
Advanced Clinical Practitioner - Oldham Urgent Care Advanced Clinical Practitioner, £55,157 to £61,850 a year plus enhancements attached to KSF and dependent upon experience.

gtd healthcare is one of the largest, not-for-profit, NHS commissioned primary and urgent care providers in North West England. We are looking to recruit additional Advanced Clinical Practitioners to work within our Oldham Primary Care Streaming Service at Royal Oldham Hospital.
The service currently runsMonday to Sunday between 08:00 and 20:00 (Subject to change to a 24 hours service).
Our team is made up of Advanced Clinical Practitioners from a variety of backgrounds including Nurses, Paramedics and Pharmacists, where shared learning, continual improvement and delivering excellence is part of our embedded culture.
Oldham is a diverse and multi-cultural community. A highpercentage of our patients face deprivation and multi morbidity. This makes fora varied role as well as an opportunity to make a real different to thecommunity. The team are passionate about providing patients in Oldham with thehighest possible quality care.
This is an excellent prospect to work in a team who has a real commitment to continuous improvement, shared learning and best practice. You will work alongside established Advanced Clinical Practitioners, GP's as well as a supportive Administrative Team.
Main duties of the job As an Advanced Clinical Practitioner working within the Oldham locality you will holistically assess and clinically examine patients with both minor and complex needs. You will have access to pathways and a broad Directory of Services (DoS) along with :
Youwill possess leadership qualities and provide support to the wider team, act asa positive role model and proactively identify areas for quality improvement.
You will have full access to our gtd learning academy and forums. This provides a great opportunity to grow and expand your clinician skills/knowledge and to excel as an ACP.
Using EMIS, you will consult with patients with undiagnosed/undifferentiated problems. You have access to the patients full clinical records and consult via telephone, video or face to face, according to patient needs.

- Referral rights to SDEC and secondary care.
- Access to technology to support telephone and video consultations (full induction and training provided).
- Facilities to see patients face to face (with support if you are expanding your scope of practice).

- Assessand holistically clinically examine patients with both minor and complex needsthrough history taking, physical examination and appropriate use ofinvestigations to confirm diagnosis.
- Plan,implement and evaluate an effective management plan for the patient, ensuringsafe discharge/referral and/or transfer to an appropriate health care service.
- Provideinformation and advice on prescribed or over-the-counter medication onmedication regimens, side-effects and interactions.
- Prioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergencycare.
- Referand discharge safely ensuring that safety-netting advice is offered.
- Triagetelephone referrals, calls from external clinicians, patients and others into arange of services appropriately as required.
- Utiliseand demonstrate sensitive communication styles, to ensure patients are fullyinformed and consent to treatment.
- Communicateeffectively with patients and carers, recognising the need for alternativemethods of communication to overcome different levels of understanding,cultural background and preferred ways of communicating.
- Anticipatebarriers to communication and take action to improve communication.
- Maintaineffective communication within the organisation and with external stakeholders.
- Ensureawareness of sources of support and guidance and provide information in anacceptable format to all patients, recognising any difficulties and referringwhere appropriate
- If you are employed in a prescribingcapacity, you must only prescribe within your scope of practice and comply withthe gtd healthcare NMP governanceregistration processes.
- You must maintain your knowledge and skillsand prescribe within your scope of clinical competencies.
- The non-medicalprescriber is required to comply with organisational NMP governance processesand adhere to local and national prescribing guidance.
- Recogniseand work within own competence and professional code of conduct as regulated bythe NMC/HPC etc. Understand own role and scope and identify how this maydevelop over time.
- Produceaccurate, contemporaneous and complete records of patient consultation,consistent with legislation, policies and procedures.
- Prioritise,organise and manage own workload in a manner that maintains and promotesquality.
- Ensureprofessional revalidation requirements are met.
- Delivercare according to NSF, NICEguidelines and evidence-based care.
- Assesseffectiveness of care delivery through self and peer review, benchmarking andformal evaluation.
- Initiateand participate in the maintenance of quality governance systems and processesacross the organisation and its activities.
- Inpartnership with other clinical teams, collaborate on improving the quality ofhealth care responding to local and national policies and initiatives asappropriate.

- Understandand apply guidance that supports the identification of vulnerable and abusedchildren and adults, and be aware of statutory child/vulnerable patient healthprocedures and local guidance.
- Takeresponsibility for own learning and performance including participating inclinical supervision and acting as a positive role mode.

- Take alead role in planning and implementing changes within the area of care andresponsibility.
- Workwith senior clinical and management teams to ensure sufficient staff ofappropriate ability, quality and skill-mix is available to meet current andfuture service delivery, that selection and recruitment processes are effectiveand that equality of treatment of the team incorporates quality HR principlesand processes.
- Contributeto the development of local guidelines, protocols and standards.
- Maintaineffective communication with those responsible for the overall commissioningand procurement process.
- Toattend and be proactively involved in professional meetings at base andcorporately.
- Takethe lead/ develop a special interest within certain areas to allow the sharingof clinical knowledge and expertise.
- Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working.
