Workingautonomously within your scope of practice, you will assess, diagnose, treat,and manage patients with a range of acute and long term conditions, playing apivotal role in delivering high quality primary care.
Main duties of the jobThe ACP or ANP will play a key role in delivering high qualityclinical care within the practice. The role involves the assessment, diagnosis,treatment, and or referral of patients presenting with undifferentiated andundiagnosedconditions. The postholder will work independently withintheir professional scope of practice, with regular debriefs and access to asupervising GP for advice throughout the day.
Main Responsibilities
- Deliver a high standard of patient care usingadvanced autonomous clinical skills supported by a broad and indepththeoretical knowledge base.
- Assess, diagnose, and manage patientconditions, initiating appropriate referrals when required.
- Manage acute illnesses, long term conditions,and minor ailments confidently and safely.
- As an independent prescriber, prescribe safe,effective, and appropriate medication in line with legislation and professionalguidelines.
- Contribute to the practices triage and urgentcare systems.
- Work collaboratively with GPs, nurses, andwider healthcare professionals to ensure seamless, highquality patient care.
- Promote health and wellbeing using healtheducation, screening, and therapeutic communication skills.
- Maintain uptodate clinical knowledge andskills, with full support from the practice for ongoing training andprofessional development.

Job responsibilitiesThe ACP or ANP will work autonomously within theirprofessional boundaries, providing high quality clinical assessment, diagnosis,treatment, and evaluation to patients presenting with both acute and long termhealth conditions. The post-holder will manage undifferentiated and undiagnosedpresentations, making complex decisions based on advanced clinical knowledgeand skills.
Working as part of a supportive multidisciplinary team, the ACP or ANP willcontribute to safe, effective, and patient centred care, supporting thepractices commitment to high clinical standards and continuous serviceimprovement. Regular debriefs and access to a supervising GP will be availablethroughout the working day.
The main responsibilities of the role include:
Clinical Care

- Assess, diagnose, and treat patientspresenting with a wide range of conditions, making independent clinicaldecisions and initiating direct referrals where appropriate.
- Manage acute illnesses, long term conditions,and minor ailments safely and effectively.
- As an independent prescriber,, prescribesafe, effective, and appropriate medication in line with the currentlegislative framework.
Triage & Urgent Care
- Work within the practices triage system toprioritise patient needs.
- Support urgent care provision by assessing andmanaging sameday or urgent presentations within scope of practice.
- Work collaboratively with GPs, nurses,pharmacists, and wider healthcare professionals to ensure continuity and high qualitypatient care.
- Contribute to multi disciplinary discussions,clinical meetings, and shared care approaches.
Health Promotion
- Promote health and wellbeing through healtheducation, screening, lifestyle advice, and therapeutic communication.
- Support patients in self management of theirconditions using evidence based strategies.

- Previous primary care experience, specifically working within a GP practice nursing team
- Experience of NHS IT System EMIS.
- Good level of communication skills both verbal and written which can be tailored to a wide range of stakeholders.
- Good level of ICT knowledge and skills.
- Good organisational skills and time management.
- Professional calm and efficient manner.
- Self-motivated, can work well independently and within a team.
- Works safely and effectively when working autonomously and in conjunction with other health care professionals
- Experience of clinical audit evaluation research and implementation of evidence based practice
- Understanding of national and local guidelines
- Understand clinical governance and risk management
- Hold one or more acedemic Level 6 diploma or postgraduate certification in long-term conditions care and or public health initiatives.
- Registered with the NMC and maintains revalidation in line with NMC requirements.
- Working at Registered Nurse Level Practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.
- Enhanced DBS check.
- Evidence of post qualifying and continuing professional development.
- Valid UK driving licence.
