Wirral Recovery Navigator: Coordinated Care & Support

Change Grow Live in Wirral is seeking a Recovery Coordinator to support people affected by drug or alcohol use. You will complete assessments, develop recovery plans and coordinate clinical, psychosocial and community support to build independence.

You will work within a multidisciplinary team, manage risk, respond to safeguarding concerns, and promote harm reduction, maintaining confidential records and joining partners across health and social services.
