Job Description

Are you looking for anexciting role providing medical oversight for community multi-disciplinaryteams working to prevent unnecessary hospital admissions for people whoseneeds are best met in their own home?

We are looking forregular experienced GPs to join our integrated team working flexibly on aMonday and/or Tuesday. The hoursavailable include 08:00-20:00 or 08:00-14:00 and 14 00.

The role will be basedat Nelson House in Hereford.

This role wouldideally suit GPs who would welcome the time to unpick complex medicalhistories and support proactive care planning; those with an interest infrailty; and those who enjoy supporting the learning and development ofadvanced practice colleagues.

Salary :- In hours rate from £46.12ph - 47.75ph,Otr of Hours rate ( 6pm-8pm & weekends) £55.34ph - £57.65ph.

Interviews are expected to be held on week commencing 9th March 2026

Main duties of the job

The GP will providetelephone and video support to the community ACPs, nurses and therapistsassessing patients in the community.

The GP will supportpatients who are managed in a GP-run virtual ward. The GP will also supportparamedics and the ambulance service for patients who do not need conveyanceto hospital but may still need some medical intervention.

Job responsibilities

JOB PURPOSE

To provide afull general practice service across different settings according to theportfolio role of the GP and the needs of the service.

Main Responsibilities

The post holderwill be responsible for seeing patients in an identical manner to regulargeneral practice work, taking responsibility for a patients care and onlyreferring back to their own GP where appropriate, consistent with ensuringevery contact counts.
