This position is open exclusively to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB), Medway Community Healthcare, and to Student Nurses and Newly Qualified Nurses.
Main duties of the jobUrgent Care Services provide short term health, therapy and care support to help patients remain safely in their own homes. The service operates 24 hours a day, 365 days a year, delivering responsive, person centred care across the community.
Key responsibilities include:
- Providing unplanned care, including end of life support, catheter care and urgent interventions that cannot wait until the following day.
- Supporting timely hospital discharge and enabling rehabilitation to maximise independence.
- Collaborating with East Kent Hospitals University Trust to develop a virtual ward, monitoring patients at home using digital tools, in partnership with consultants, specialist GPs and advanced practitioners.
- Working in integrated neighbourhood teams alongside GPs, mental health professionals, social workers and voluntary services to deliver joined up care.
- Level 6 Degree or equivalent qualification, or equivalent knowledge plus a diploma in nursing.
- Registered health care professional with the Nursing and Midwifery Council.
- Recent experience in healthcare through education, work experience or previous employment; ideally a newly qualified graduate nurse.
- Full UK driving licence or a full driving licence issued by an EU country; driving is required for the role.
- Customer focused approach, providing the best possible experience for patients, clients and families.
- Proficiency in IT systems and databases used in clinical practice.
- Ability to signpost clients and families to alternative services to support behaviour change and improve health.
- Knowledge of up to date clinical practice, evidence based procedures and clinical effectiveness frameworks.
- Awareness of developments within the NHS (advantageous).

Salary: £32,073 to £39,043 per annum pro rata.
