Transitions Liaison for SEND & EHCP Pathways

Exeter College is seeking a Transitions Link Officer to be the vital link between schools, families and the College, ensuring that learners with EHCPs and additional learning support needs experience a smooth transition into further education.

You'll gather and share information, support transition planning, act as an ambassador for the College, and help families understand available support and pathways.
