Job Description

To provide leadership and oversight to care staff within supported living services to ensure safe, effective and person-centred support delivery.

To support the Manager in ensuring the smooth day-to-day running of supported living homes and maintaining high standards of care and compliance.

To oversee quality assurance, medication management and staff practice within the service.

To act as a key point of contact between care staff, management, external professionals and the organisation's contact centre.

Responsibilities and Duties of the Job

Role-Specific Duties

  • Lead and coordinate the day-to-day running of supported living homes to ensure services operate smoothly and safely.
  • Provide leadership and guidance to care staff, ensuring high standards of person-centred support are delivered.
  • Supervise and support care staff through regular supervisions, mentoring and performance monitoring.
  • Monitor staff practice and ensure support is delivered in line with care plans, risk assessments and organisational policies.
  • Oversee medication management within the service, ensuring safe administration, accurate MAR chart completion and adherence to medication policies.
  • Conduct regular quality assurance checks, audits and spot checks to ensure compliance with organisational standards and regulatory requirements.
  • Support the Manager in maintaining compliance with CQC Fundamental Standards and safeguarding responsibilities.
  • Ensure all support documentation, care plans and risk assessments are accurate, updated and reflective of individuals' needs.
  • Support residents to attend appointments, reviews and community activities where required.
  • Liaise with healthcare professionals including community nurses, hospitals and local clinical teams to ensure coordinated care.
  • Work with local authorities and other professionals to support care reviews and assessments.
  • Support coordination between operational teams and the organisation's contact centre to ensure smooth case management and effective communication.
  • Provide support in coordinating referrals, resident placements and communication relating to supported living services.
  • Respond appropriately to incidents, accidents and safeguarding concerns, ensuring reporting procedures are followed.
Working with Others
  • Develop effective working relationships with colleagues, residents, families and external professionals.
  • Liaise with healthcare professionals, including community nurses, hospitals and specialist teams, to ensure residents receive coordinated care.
  • Work collaboratively with management to ensure services operate effectively and meet organisational and regulatory standards.
  • Maintain effective communication with the contact centre team to support case coordination and referrals.
  • Promote a positive team culture, encouraging teamwork, accountability and professional conduct among staff.

Person Specification
  • NVQ Level 3 in Health & Social Care (or equivalent)
  • NVQ Level 4/5 in Leadership for Health & Social Care
  • Medication administration training
  • Safeguarding training

Location

The service's premises, but you may be required to work from other locations at the discretion of the company and with appropriate notice.

Working Hours

Available Shifts - day, evening, night, weekend or rota. Duties also include providing care on a shift or schedule to ensure teams meet frequency and daily support needs. Roles may involve single hours, multiple hours or full everyday rotation. These flexible hours give you the opportunity to work. Working hours are regulated under the NHS Staff Regulations 2022. The Role covers a variety of shifts - day, evening, night, weekend or rota.
