We are based in Yeadon Health Centre, but we cover Rawdon, Yeadon, Guiseley, Menton, Pool in Wharfedale, Otley and the surrounding area.
Our community nursing teams deliver planned care in patients homes, helping people to remain independent and supported within their local communities. We provide a wide range of clinical interventions, including (but not limited to):
- Bowel and bladder management
- Wound care (including leg ulcer management)
- Pressure ulcer prevention and management
- Palliative and end-of-life care
- Use and management of enteral feeding tubes
- Safe administration of medication
Through our work, we enable early discharge from hospital, reduce avoidable hospital admissions, and promote patient independence and wellbeing at home.

Main duties of the jobYou'll be part of a compassionate and highly skilled multidisciplinary team, working alongside District Nurses, Community Matrons, Occupational Therapists, Physiotherapists, Neighbourhood Clinical Assistants, Therapy Assistant Practitioners, and Self-Management Facilitators to deliver truly holistic, person-centred care.
You'll also collaborate closely with our Citywide Services, including the Falls, Continence, Urology and Colorectal Care, and Palliative and End of Life Care teams, as well as GP services, ensuring the best possible coordination and continuity of care for patients across West Leeds.

- Providing safe, evidence-based clinical care in patients homes.
