37 hours per week,

£37,280-£46,142 per annum. Starting point will be determined in line with Progression FrameworkA relocation package up to £8,000 will be considered for this post.

Our "Strengths-Based Model of Practice" supports a multidisciplinary approach to enabling and supporting people who live with issues caused by disability, frailty, and ageing.

They will be based within the Early Intervention and Prevention Hub, which includes the short-term social work, community support workers and the front door response in Adult Social Care. The team supports a multi- disciplinary approach, with a focus on adults who have multi-faceted complex needs and cognitive decline. This service focuses on outcomes for individuals, maximising individual potential and minimising risk to independence, with an emphasis on choice and control.

  • A recognised social work qualification and registration with the Care Council for Wales is essential.
  • The ability to greet customers through the medium of Welsh is a requirement for this post.
  • Protecting children, young people or adults at risk is a core responsibility of all council employees.
  • An Enhanced Children and Adults Barred list criminal records check by the Disclosure & Barring Service (DBS) is a requirement for this post.
  • A valid driving licence is requirement for this post.
  • The council's Hybrid Working Policy applies to this post. This provides a framework for establishing how you will undertake working hours between your home and the office.
