Job Description

The role will include working across Social Care, all ages, and will involve multidisciplinary and multi-agency partnership working. Social Care Assessors may be deployed to or required to undertake work within any of the teams within the department, commensurate with relevant skills and experience. This will include allocation to other teams, to further develop knowledge and skills in other specialist areas, to ensure the service can operate flexibly to meet service demands.

The post holder will work as part of a team with a range of other professionals. Working in a changing environment, the post holder will assist the Department in meeting its responsibility to implement the Social Services and Wellbeing Wales Act (2014) and achieving the aim of closer working relationships with Health and other partners to deliver improved outcomes across the social care sector. They will manage an allocated caseload which includes Assessment and Social Care interventions, escalating those that are more complex and require a higher level of professional input to a qualified Social Worker / Occupational Therapist.

To maximise the independence, wellbeing and safety of adults, children and carers of Anglesey who have care and support needs, the post holder will undertake assessments of need with adults, children and carers using the Department's procedures, and where required, undertake separate assessments of the needs of the person's carers. They will develop and design care and support plans, other appropriate plans (e.g. education plans) and recommend any external support within budgets, ensuring the best use of resources, and ensuring necessary approval when required (e.g. Resource Panel Applications).

They will plan creatively with individuals, maximising assets, reducing risks and reducing dependency on long-term service provision. This will include the provision of information and advice, supporting people to access universal community resources, prescribing community equipment and adaptations, and accessing statutory care and support services as required.
