Social Care Assessment & Advocacy Officer

Merthyr Tydfil County Borough Council is seeking a qualified Social Care practitioner to join the busy Duty Team and undertake adult care assessments under the Social Services and Well-being Act 2014.

The role covers duty rota, information provision, safeguarding, advocacy awareness, and organising services including packages of care and day services, within a 37-hour week. The post requires structured enquiry handling and robust documentation, with a focus on independence and positive outcomes.
