Job Description
Senior Registered Nurse

This post is to support the Hospital at Home service which is a collaboration with the acute trust to deliver hospital standard treatment within patients' homes. The Hospital at Home philosophy is providing acute clinical care at home which would otherwise be provided in a hospital. This service is part of the Urgent Care Services which provides a two hour response to patients in crisis at home. Our aim is to prevent unnecessary hospital admission and facilitate early discharge from the acute hospitals. We also provide unscheduled care out of hours for patients who may be at the end of life, or experiencing an urgent need for healthcare. This may include treating acutely unwell people that would otherwise require hospital admission, and also support hospital discharges. The service is available 24 hours a day 365 days a year. Nurses use a wide clinical skills base to provide hospital level care at home. This includes IV therapy, end of life symptom control and management via syringe driver, catheterisation skills, phlebotomy, wound management, risk assessment, as well as general observations and fundamental nursing care. This service provides nursing care within patients' homes and covers West Kent. Driving is essential to this post.

Main duties of the job
  • Assessment, planning, implementation and evaluation of holistic care.
  • Development of integrated care plans in partnership with patients.
  • Ordering equipment and sending onward referrals as necessary.
  • Acting as a lead and senior practitioner to support junior staff with assessments and reassessments of service users and/or pathways with complex needs across the relevant field.
  • Actively working as a member of the multi professional team to provide high quality care to patients.
  • Undertaking a range of competent clinical interventions and identifying any deterioration in patients' condition, taking appropriate action.
  • Managing a caseload of patients on a daily basis consisting of patients with complex needs.
  • Supporting in duty role and completing triages.
  • Supporting the Clinical Lead in the management of the team.
  • Supporting and making recommendations to senior managers to prioritise and focus efforts to drive forward and achieve business objectives, service improvement strategies and statutory requirements.

Person Specification Qualifications
  • Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing.
  • Registered health care professional with the Nursing and Midwifery Council.
  • Evidence of experience of managing complex caseloads and supporting/mentoring others.
Experience/Knowledge

  • Knowledge of up-to-date clinical practice by using up-to-date skills and adhering to evidence-based procedures and clinical effectiveness frameworks.
  • Fully understand the role and remit of a health care worker.
Skills & Abilities

  • Know when you are within your own limits and when to elevate.
  • Able to use IT systems and databases.
  • Have an awareness of developments within the NHS would be an advantage as well as an understanding of National Service Frameworks and NICE guidance.

  • Need to be able to work well under pressure and be able to organise your own work.
