Job Description
Senior Pharmacist (Hospital at Home) 
Closing date: 23 June 2026
Location: Middlesbrough, Redcar and Cleveland.
Main duties of the job- Lead and oversee clinical pharmacy input to the Urgent Community Response (UCR) and frailty virtual ward team to deliver care to the virtual ward and Urgent Care Response teams.
- Contribute to patient care both within the unit base setting and via patient home visits where appropriate.
- Lead on medication provision and delivery across organisational boundaries.
- Complete medication reviews, medicines reconciliation and pharmaceutical care plans for patients admitted to the virtual ward.
- Work as an independent prescriber, optimising medicines and promoting appropriate, evidence based and cost effective prescribing and deprescribing.
- Use clinical skills in investigations, analysing and implementing actions for medication related incidents within the virtual ward.
- Provide specialist clinical pharmacy knowledge to health and social care professionals on the safe prescribing, handling and administration of medicines.
- Must have access to a vehicle for work purposes.
- Qualified to a degree level (MPharm or equivalent).
- Registered with the General Pharmaceutical Council or Great Britain as a practising pharmacist.
- Postgraduate Diploma in clinical pharmacy, or equivalent.
- RPS Faculty membership.
- Professional knowledge acquired through vocational degree in pharmacy.
- Significant pre registration training and experience.
- Significant post qualification experience in a relevant role, practising as a pharmacist in hospital or primary care.
- Good all round clinical knowledge and practical clinical skills.
- Experience of supervising staff and leading a team.
- Ability to work autonomously and within a team.
- Experience of community services.
- Worked as a clinical pharmacist at a high level.
- Creating, developing and reviewing protocols and/or guidelines.
- Supervising and undertaking audit and reporting results appropriately.
- Experience and involvement in service development, teaching, tutoring or mentoring pre registration pharmacists.
- Ability to travel independently throughout the area.
- Experience prescribing in a specialist area.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
Salary: £57,528 to £64,750 a year pro rata.
