Senior Pharmacist - Hospital at Home

An exciting new role has been created for a motivated and patient centred, senior clinical pharmacist to utilise their clinical skills and work with multidisciplinary colleagues across the Integrated Care System (ICS). You will provide co ordinated care to patients, reduce hospital re admissions, support safe discharge, transfer of care, and improve medicines safety in Worcestershire's new Hospital at Home service. A confident communicator with strong organisational skills, who thrives in an MDT environment, you will be instrumental in supporting and developing this new service, to ensure high quality care for our patients. Main duties of the jobYou will need to be a confident decision maker, with clear leadership skills, excellent pharmaceutical knowledge and technical skills and a sound understanding of pharmacy service and hospital service. Main duties include:
- Lead, deliver, develop, and assess the value of clinical pharmacy services to support Hospital at Home service.
- Support the development of good practice and safe medicines management and optimisation systems across Herefordshire and Worcestershire Integrated Care System (ICS).
- Work in collaboration with different organisations across the IC to provide highly effective clinical pharmacy services to patients within Hospital at Home service.
- Develop and work within appropriate governance structures for the designated clinical service liaising with ISC wide stakeholders as needed.
- 27 days leave plus bank holidays, increasing up to 33 days with long service
