Here is an exciting opportunity for you to join us as a Senior IDT Nurse Assessor.
The Integrated Discharge Team works with patients and families with complex needs and conditions, supporting them in the discharge process in order for a smooth transition in to the community. We aim to anticipate, co-ordinate and join up health and social care needs for patients at a high risk of unplanned admissions to hospital.
We provide guidance and professional support to the patient and the patient's families, and works with them in partnership to make sure the patient gets the most appropriate plan in place for discharge, whilst ensuring that interventions are not duplicated. This process, improves the quality of care as well as improving patient satisfaction.
The person is at the centre of holistic care planning working within Southend hospital alongside other organisations such, social care and the voluntary sector.
Our aim is to promote and work towards an Integrated Discharge Team way of working as discussed within the NHS Long Term Plan

Main duties of the jobThe Integrated Discharge Team support patients with complex healthcare needs, who may require long-term health and social care planning or a short term intervention following a change of condition or acute illness.
The role involves the assessment of patient needs and planning and facilitation of safe and robust discharges working alongside the hospital MDT and our external system partners to ensure that patients are discharged from the acute setting aiding to maintain patient flow throughout the acute trust.
The Senior Nurse Assessor will also manage junior members of staff with support of the Operational Lead, to provide effective leadership and good working relationships

Person Specification Qualifications- RGN / Minimum 4 years clinical and above
- 2 years senior level or above (band 6 and above)
- Excellent communication skills / evidence of MDT / team working
- Been part of an MDT team / Discusses understanding surrounding MDT working
