Senior Discharge Navigator Lead Care Coordination

Royal London Hospital is seeking a Senior Discharge Navigator and Lead Screener to join the Transfer of Care Team. Based on the ward, you will screen referrals, lead discharge planning for complex cases and coordinate with multidisciplinary teams and external agencies to ensure timely, safe discharges.

You will advocate for patients, identify needs, link to community services, and support home-first discharge goals while following current government guidelines.
