Overview

Senior Community Nurse role with Bradford District Care NHS Foundation Trust. Closing date: 17 September 2025. This is a predominantly clinical role (90% clinical, 10% non-clinical).

Main duties of the job

The Senior Community Nurse post is predominantly clinical, with a 90% clinical and 10% non-clinical split. Post holders will use their knowledge, skills and expertise to assess, plan, implement, monitor and review patient care. They will develop competencies and clinical knowledge with patients, carers, colleagues, students and the wider multi-disciplinary team.

Post holders will also support the District Nurse Caseload holder with caseload management and have a key role in communicating important information through the team.

Person Specification Qualifications
  • 1st level registered Nurse
  • 2 years post-registration experience
  • Postgraduate qualification in a specialist field or equivalent experience
  • Professional degree or diploma in relevant field
  • Maintains a portfolio of CPD in line with regulatory body standards
  • V300 holder
  • Long term condition experience or willingness to undertake
  • Community nursing experience
Skills
  • Experience of MDT working
  • Understanding of Quality and Safety agenda (Clinical Governance)
  • Good organisational and presentational skills
  • Understanding of Risk Management Issues
  • Ability to travel between multiple sites in fulfilment of job role
Experience
  • Working experience of complex care management
  • Experience of effective multidisciplinary team working
  • Experience of budgetary and financial management
  • Experience of managing and developing staff from different professions
