This is an exciting opportunity to work in a fast paced and dynamic team, within the Southeast Division of Hampshire and IOW trust. We have the opportunity for a full time 37.5-hour position for a band 6 clinical lead. Contributing to the prevention of hospital admissions under a strong and robust frailty model, where Patients are cared for on a virtual ward in their own home, ensuring their needs are met and they are monitored during an acute episode of illness. If you have existing nursing skills which you feel are not used to their full potential, and you feel your job is repetitive with the same tasks every day. Then look at this role, it provides a true variation of demand, it will make you utilise skills you maybe don't get to use. Every day you will look forward to coming to work, knowing you are preventing a hospital admission and supporting someone to remain at home.
Experience in community care is desirable, however full training can be provided, and ongoing post Reg training is available including the physical assessment course, diagnostics and independent nurse prescribing.
