Senior Community Mental Health Nurse - Dementia Older Person 
Band 6, permanent full time (37.5 hours per week with flexible 7 day coverage). Job reference A.
Site: Bolitho House, Town, Penzance. Salary: £39,959 - £48,117 per annum, payable yearly.
Main duties of the jobThe post holder will provide assessment, treatment and care coordination for:
- People living with dementia.
- Adults over 75 with a range of mental health needs.
The role is community based and requires frequent travel across the county, including sites that may not be accessible by public transport.
Responsibilities- Manage a caseload under the Care Programme Approach, placing the individual at the centre of care.
- Work with statutory and non statutory agencies to develop and deliver personalised care plans.
- Provide clinical supervision as required.
- Collaborate with multidisciplinary and multi agency teams.
- Develop and maintain relationships with carers, family members and community services.
- Registered with the Nursing and Midwifery Council (NMC).
- Post graduate qualification or equivalent experience in psychological therapy (e.g., CBT).
- Substantial experience managing a busy caseload of clients with dementia and mental health problems, including frailty related to older age.
- Experience providing clinical supervision.
- Experience working within multidisciplinary and multi agency environments.
