Senior Community Facilitator - Lead Person-Centred Support

Corlann - West Region in Strokestown is seeking a Senior Community Facilitator to lead frontline support for individuals with Intellectual Disability, Autism and Complex Behaviours, delivering person-centred care and implementing Behaviour Support Plans.

The role involves acting as a key worker, promoting independence and supporting person-centred plans across Strokestown, Elphin, Tulsk, Ballinagare and Ballaghaderreen.
