Senior Case Worker - Independence & Care Planning (Adults)

Hampshire County Council is seeking a Senior Case Worker to support adults with disabilities, learning disabilities and mental health needs. You will manage a diverse caseload, guide colleagues, and model best practice to promote independence and wellbeing.

You will review care plans countywide, provide guidance, and signpost to community resources. The role requires a proactive, compassionate approach and strong teamwork within our Younger Adults Review Team.
