Senior Case Worker - Hospital Discharge & Care Coordination

Hampshire County Council is seeking a Senior Case Worker to join the Hospital teams at Winchester Hospital. You'll conduct assessments and support hospital discharges in partnership with social care and health professionals.

This role focuses on face-to-face work with individuals in Winchester and local community settings, with opportunities to develop through our apprenticeship pathways and a strong emphasis on collaboration and safeguarding.
