Senior Advanced Nurse Practitioner

We are seeking highly experienced Advanced Practitioners to join our Ageing Health clinical team within Forth Valley Royal Hospital.
This role is appropriate to those Advanced Practitioners who are qualified and annotated on the national and local workforce database as an advanced practitioner. Applicants must have knowledge and significant experience of managing patients with high clinical frailty scores and undifferentiated complex care presentations, demonstrating competency in the breadth and depth of diagnosis and treatment.
As part of a multi-professional team the post holders will be expected to liaise with patients and carers regarding clinical conditions and care planning, at times sharing highly sensitive information.
Applicants must experience communicating realistic medicine values and engaging in shared decision-making, therefore excellent communication skills are essential. Evidence of clinical and professional leadership, service development and experience of facilitating learning are also essential.
The role will involve working across organisational boundaries and in partnership with other health and social care teams. Successful candidates must demonstrate a genuine passion to improve the experience of older and frail patients during acute admissions.
To carry out these duties the successful candidate must be a member of the Protecting Vulnerable Groups (PVG) Scheme.
Essential Credentials for band 8a:- Qualified Advanced Practitioner annotated on local and national registers
- Significant Advanced Practice Experience working with older adults
- Highly developed leadership skills
- Excellent Communication skills
