SEN Transition Navigator

Education Partnership North East is seeking a dedicated SEN Support Officer to lead early transition planning for students with SEND. You will collaborate with schools to assess needs, design targeted support, and prepare learners for college life, contributing to EHCP processes and multi-agency engagement.

You will work across campuses with opportunities to develop expertise in SEN support, college systems, and data management, delivering positive outcomes for learners and families.
