By working together with our different Network teams, we use our combined skills to provide a service that is joined-up, holistic, proactive, and personal for the patient.
We are fortunate that all our practices are located close to the beautiful city centre of Bath, with excellent transport links, schools, places of interest for the whole family, and an abundance of restaurants and bars.
Our Network teams include:
- Enhanced Care Visiting paramedic for frail housebound patients.
- Nursing associates for in-clinic work and visiting housebound patients with Long Term Conditions.
- First Contact Physiotherapy Practitioner service.
- Pharmacy team comprising Clinical Pharmacists, Pharmacy Technicians, and Care Coordinators.
- Social Prescriber.
Following the release of ARRS GP information and funding, we are seeking newly qualified (within 2 years and not having held a substantive post), enthusiastic, forward-thinking GPs to join Keynsham and Bath Independents Primary Care Networks on a permanent basis. Applications for 2-6 sessions will be considered, with a start date as soon as possible.
You will be required to work across three practices (three sites), supporting our practices. Our practices are training practices. We are highly supportive PCNs, understanding the importance of a good work/life balance with an open-door ethos.
Your duties will include:
- Face-to-face and telephone consultations
- Patient home visits and nursing home visits
- Checking and signing repeat prescriptions
- Administration tasks such as dealing with queries, patient test results, referral letters (NHS/private), emails, and other correspondence in a timely manner
- Managing clinical aspects of patient care, including investigations, reviewing results, and making referrals to secondary care or other providers as appropriate
Keynsham PCN is a medium-sized PCN with a patient list of 28,000. We have a significant workforce that offers excellent opportunities for in-house multidisciplinary team working. Our team includes Advanced Nurse Practitioners, Clinical Pharmacists, Pharmacy Technicians, Paramedics, First Contact Physiotherapists, Nursing Associates, support staff, and more, enabling most patient needs to be met on their doorstep.
Bath Independents Primary Care Network (PCN) comprises three practices: Batheaston, Fairfield Park, and Widcombe Surgeries. We serve a population of 30,000, working collaboratively as a team to deliver high-quality patient care.
Job responsibilitiesMain Tasks:
Clinical:
- Recording clear, contemporaneous IT-based consultation notes to established standards
- Providing safe, evidence-based, cost-effective, individualized patient care within the surgery, patient's home, or other settings
- Assessing patient needs from physiological and psychological perspectives, planning clinical care, and referring patients to other services as needed, following guidelines
- Performing and interpreting investigatory procedures, including laboratory and radiology tests
- Prioritizing health problems and intervening appropriately in urgent or emergency situations, including initiating emergency care
- Ensuring appropriate follow-up of patients
- Supporting health promotion and self-care strategies
- Supporting delivery of anticipatory care plans
- Maintaining accurate health records and working within your competence and professional codes of conduct
- Prescribing independently within your scope of practice, following guidelines
- Managing prescriptions and reviewing medications according to policies and guidelines
- Staying informed of developments in clinical practice
- Ensuring compliance with practice policies and relevant legislation, including safeguarding
- Experience in urgent care or primary care settings
- Experience managing a range of clinical conditions
- Experience conducting home visits
- Ability to evaluate clinical practice safety and effectiveness
- Understanding of evidence-based practice
- Ability to work autonomously with support when needed
- Experience in triage
- Knowledge of QOF and public health issues
- Awareness of wider health issues and health promotion strategies
- GMC registration
- MBBS or equivalent medical degree
- MRCGP
