We are a practice of 19,000 patients across 2 sites in Ware (Church Street Surgery) and Hertford (Castlegate Surgery), Hertfordshire.
We are dynamic, supportive and forward-thinking team. We are a teaching and training practice, with partners teaching GP trainees and involved in Cambridge Medical School student teaching.
Main duties of the job- In accordance with the practice timetable, as agreed, the post-holder will make him/her-self available to undertake duties including surgery consultations, telephone/video consultations and queries, paperwork and correspondence in a timely fashion
- Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the practice or external organisations
- Assessing the health care needs of patients with undifferentiated and undiagnosed problems
- Effectively manage own time, workload and resources
- Screening patients for disease risk factors and early signs of illness
- Developing care plans for health in consultation with patients and in line with current practice disease management protocols
- Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
- Recording clear and contemporaneous consultation notes to agreed standards
- Contributing to QOF requirements and recording data appropriately
- Contributing to PCN requirements and recording data appropriately
- Compiling and issuing computer-generated acute and repeat prescriptions
- Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate and review as and when required

- GMC Registered MBBS or equivalent medical degree Inclusion on the Performers List Medical Indemnity Experience with British Health Care and Primary Care Systems Enhanced DBS Check
