Job Description

We value continuity of care and have personal lists for all Partner and Salaried GPs.

Responsibilities
  • Provide 15 minute appointments.
  • Meet with the team for mid-morning coffee / catch up.
  • Bring your most vulnerable patients to our fortnightly MDT led by a Care Coordinator.
  • Be part of our on-the-day clinical team, working closely with our care navigators.
