RQ (JS) - Social Worker

About the Role
We are looking for a committed and compassionate Social Worker to join our team, supporting residents to live independently and safely within the community. This role focuses on delivering high-quality, person-centred care through holistic assessments, effective care planning, and strong partnership working.

• Carry out holistic assessments of residents' needs, identifying risks and ensuring they are effectively managed
• Develop, implement and review personalised, strengths-based care and support plans to achieve positive outcomes
• Provide practical and emotional support to help residents remain living independently in their communities
• Respond to customer enquiries in a timely and professional manner, offering accurate information and guidance
• Support individuals involved in safeguarding processes, ensuring a person-centred approach and enabling them to have their voice heard
• Work collaboratively with colleagues, families, and partner agencies to deliver coordinated care and support

• A proactive and empathetic approach to supporting vulnerable individuals
• Strong assessment, care planning, and communication skills
• Ability to manage risk and make informed decisions
• Commitment to delivering person-centred and strengths-based practice
• Experience of working within safeguarding frameworks.
