RPN: Mental Health & Community Care (ACT Team)

Ontario Shores Centre for Mental Health Sciences in Whitby, Ontario seeks a Registered Practical Nurse for the Durham ACT team. Reporting to the Clinical Manager, you will join a multidisciplinary team and provide assertive community treatment to clients with severe mental illness.

Responsibilities include mental health and physical assessments, developing treatment goals with clients, families and the team, delivering medications, monitoring adherence, and supporting daily living activities.
