We have an exciting opportunity for a Release and Transfer Nurse to join our Primary Care team at HMP Pentonville, a category B Prison located in North London. Hours are Mon - Fri 10am -6pm. Bank holidays are required.
SalaryThis position is full time, you'll receive an annual salary up to £55086 per annum FTE, depending on experience.

- Manage the assessment of health needs, planning, delivery and evaluation of care for the prison population, including complex patients and clinical risk assessment.
- Provide care and planning for adult male prisoners from the point of reception until the day of release or transfer to another establishment.
- Work collaboratively with the patient and any other agencies to support a safe transition back into the community or another establishment.

Although not essential, ideal applicants would have experience in a hospital, community or prison environment.
