Job Description

Based at our Hospice in Winsley. This is a Part Time position, working 3 days a week.You will plan and deliver personalised exercise programmes to patients across the In-Patient Unit, Day Services, and Outpatient settings

Job responsibilities

Key Duties and Responsibilities:

  • Exercise Delivery: Plan and deliver personalised exercise programmes to patients across the In-Patient Unit, Day Services, and Outpatient settings.
  • Holistic Care: Provide compassionate, rehabilitative palliative care that supports physical, emotional, and psychological wellbeing.
  • Collaborative Working: Work closely with physiotherapists, occupational therapists, and the wider multi-disciplinary team to deliver coordinated, person-centred care.
  • Assessment & Planning: Assess patients' needs and develop tailored rehabilitation plans in partnership with families, carers, and healthcare professionals.
  • Non-Pharmacological Support: Deliver evidence-based, non-pharmacological interventions to help manage symptoms and improve quality of life.
  • Team Supervision: Support, supervise and mentor assistant staff, students and volunteers involved in exercise and rehabilitation services.
  • Record Keeping & Audit: Maintain accurate clinical records and contribute to service audits, evaluations, and ongoing improvements.
  • Service Development: Contribute to the review and improvement of rehabilitation services, including policy development and identifying opportunities for innovation.

EXERCISE REHABILITATION PRACTITIONER Band 5 £29,970 - £36,483 per annum (pro rata)

    • BSc in a health or exercise-related subject (e.g. Sport Science, Exercise Physiology)
    • Evidence of continued professional development and attendance at exercise/rehabilitation training courses
    • Registration with a relevant professional body
    Experience
    • Can demonstrate confident awareness and interest of rehabilitative palliative care
    • Experience supporting people with physical health conditions in a clinical setting (e.g. hospital, hospice, or through placements/work experience)
    • Excellent written, verbal and interpersonal skills, with the ability to build positive relationships
    • Strong admin skills with the ability to prioritise workload, manage time effectively and work independently
    • Experience delivering education or training to groups individuals
    • Experience working with volunteers and/or supervising staff or students
    • Experience supporting individuals with multiple health conditions or comorbidities
    • Experience delivering exercise programmes to people with chronic conditions or at end of life
    • Experience providing services in a community setting
