Job Description
Referral & Allocations Partner - Dorchester

As a Referral and Allocations Partner, you will receive direct referrals and enquiries, providing a single point of contact for the Social Reablement and Crisis Support Service. You will work alongside partner agencies to ensure people accessing the service receive the correct support in a timely manner.

Salary: £26,607 pa for 40 hours per week

Key responsibilities include:

  1. Having in-depth knowledge of person-centred, asset-based support planning and risk assessments, including safeguarding children and adults.
  2. Being aware of factors that put people at risk of homelessness, hospital admission, or delayed discharge.
  3. Being skilled in IT and updating case management systems.
  4. Having experience working with a diverse range of people and in partnership with others.
  5. Possessing a Level 3 NVQ qualification or equivalent work-level experience.

The appointed Referral and Allocations Partner will:

  1. Work person-centred at all times, identifying what is important to individuals and ensuring appropriate interventions.
  2. Deliver a 'no wrong door' approach, providing supported referrals to other services where more appropriate.
  3. Work directly with the Social Reablement team to allocate cases effectively.
  4. Collaborate with statutory and non-statutory services, including hospitals, CMHTs, substance misuse teams, healthcare professionals, social prescribing initiatives, and early help services, to provide a coordinated, holistic service involving advocacy and crisis response.
