What you will do
  • Caseload Management: Support a caseload of individuals experiencing drug and alcohol dependency, delivering structured and person-centred care.
  • Assessment & Planning: Conduct comprehensive assessments and develop tailored recovery plans.
  • Intervention Delivery: Provide one-to-one interventions and facilitate harm reduction strategies.
  • Progress Monitoring: Track service user progress and adapt support plans to promote long-term recovery and relapse prevention.
  • Multi-Agency Working: Liaise with partner organisations to ensure coordinated care and access to services.
  • Risk & Safeguarding: Identify and manage risks, contributing to safeguarding processes for adults and children.
  • Health Promotion: Encourage physical and mental wellbeing, supporting engagement with healthcare services.
  • Compliance: Maintain accurate records and ensure adherence to organisational policies and procedures.
