Recovery & Housing Case Worker

Connection Support is looking for a compassionate Case Worker to help people move from hospital into independent living in Headington, Oxford. You will work in a person-centred way to identify goals, assist with housing, benefits, health services, and community supports, and collaborate with hospital teams and housing providers.

The role is 37.5 hours per week, permanent, with a salary of £27,418-£28,541 per annum, dependent on experience.
