Job Description
Job Summary 
We are looking for a Clinical Reconnect Practitioner to join our team in Humberside.
Salary & HoursFull time. Salary £43,609 to £49,175 depending on experience.
Monday - Friday: 9am - 5pm.
Location: Humberside Region & Prison sites (HMP Hull & HMP Humber).

- Work autonomously in prison, community, and police detention cells to identify and respond to requests for screening and assessment of people with identified health needs leaving the prison establishment within a 12 week period or less.
- Take referrals for service users who are leaving the prison establishment within 12 weeks or up to 28 days post release, aiming to reduce reoffending by addressing health and social needs and ensuring onward referrals for continuity of care.
- Use validated tools to assess for identified health needs and utilize information to inform decision making and facilitate diversion within and out of the criminal justice pathway, creating individualised support plans and preparing accurate written reports in line with quality standards.

- Qualified Registered Mental Nurse, Mental Health Social Worker, or Occupational Therapist with current registration with the relevant professional body.
- Substantial evidence of continuing professional development and post registration training.
- Experience of working face to face with patients who have complex mental health conditions.
- Awareness of psychosocial interventions available to service users and how to refer into service.
- Awareness of substance misuse services and available support.
