We have a fantastic opportunity for a nurse with experience in community nursing to join our Rapid Response Nursing Team in Derby City as a Senior Community Nurse.
Our service provides acute care in patients' own homes and residential care home settings. We promote a self management approach to patients with their heath needs, therefore a key priority is to offer health education at each visit.
Our service covers 7 days per week, 08 30 to support patients with urgent nursing needs.
The successful applicant will develop specialist nursing assessment skills to enable them to provide a comprehensive assessment to a variety of complex patients, both via telephone triage and clinical patient home visits.
You will be well supported in the role by a skilled multi disciplinary team and have the opportunity to participate within an in service training programme.
Travel across Derby City and flexibility are essential for this post.
Main duties of the jobResponsible for all aspects of band 6 role including:
- Triaging, assessing patient priority and allocating visits in accordance with priority and ensuring we meet urgent community response times;
- Supporting new starters, junior staff and student nurses with achieving clinical competencies and confidence;
- Delivering high quality clinical care and providing support with complex patient management and coordination of care;
- Supporting quality, audits and data analysis;
- Staff leadership including probationary meetings, 1 1's and appraisals.

Person Specification Education, Qualifications and Training- NMC registered nurse
- Relevant nursing degree or equivalent experience
- Required to undertake a leadership development programme
- Recent and relevant community experience
- Experience of long term conditions
- Proven clinical expertise, knowledge and experience of implementing service improvement
- Proven managerial skills and experience of change management
- IT and administrative skills
- Leadership and organisational skills
- Expected to travel around various areas of Derbyshire County to carry out required duties
