The right candidate will be supported by highlyskilled and experienced clinical leads delivering pharmacy support to oureight partnership GP practices within Mid Dorset.

The role is also only open to drivers with access to their own vehicle as you may need to travel within the Mid Dorset PCN area.
Main duties of the jobThe post holder will work within theirclinical competencies to provide pharmaceutical support to all practices acrossMid Dorset PCN and work closely withmembers of wider healthcare teams to improve care and support safe andeffective prescribing.
The post holderwill work as part of a multi-disciplinary team in a patient-facing role. Theywill take responsibility for areas of medicines optimisation within the PCN andundertake structured medication reviews and support long term conditionmanagement to proactively manage patients with complex polypharmacy.
The post holder will need to be proactivelyinvolved in quality improvement and audit as well as managing some aspects ofthe Quality and Outcomes Framework.
The post holder will be supported to helpdevelop their role and work to improve the care and outcomes for the patientsacross the PCN. This will include becoming and/or operating as a non-medicalprescriber.
To play a pivotal role in the delivery ofhigh-quality primary health care within the PCN.

Job responsibilitiesClinical and structured medication reviewsfor patients with single or multiple long term conditions where medicinesoptimisation is required.
To be involved in multidisciplinaryclinical meetings within the PCN where appropriate.
To deliver patient facing and telephoneconsultations, signposting to other services and/or healthcare professionalswhere appropriate, while working as part of the wider multi-disciplinary team,within scope of practice and limits of competency.
Implement changes to medicines in line withMHRA alerts, product withdrawal or shortage and other local or nationalguidance.
Maintain full and complete records of allpatient contacts using appropriate clinical templates and coding.
To undertake a proactive role in audit andquality improvement implementing recommendations where appropriate.
Provide independent information to patientswith regard to medicines and prescribing changes, initiating further supportfrom other healthcare professionals where appropriate.
Participate in review and setting ofpolicies relating to medicines management and prescribing and help ensurepractices prescribe in accordance with local guidelines and formulary.
Toidentify areas of clinical risk at network level and make recommendations tosupport the introduction of new working practices that will optimise thequality of prescribing and make more efficient use of network resources.
To monitor network practices prescribingagainst the local health economy prescribing quality scheme targets and makerecommendations for GPs or amend within scope of practice, based on findings.
To maintain own clinical and professionalcompetence and be responsible for own continuous professional development.
Work with practices to ensure full compliancewith Care Quality Commission standards for safe and effective care.
Work with care home staff to improve safety of medicines ordering and administration.
To support the Frailty and Paramedic team with pharmacy issues.
To work closely with the PCNteam in establishing good working relationships through transparency and good organisationalskills.
Attending PCN and practicemeetings when required to do so and to always represent the PCN with the utmostprofessionalism.
Provide support to colleaguesand the management team on projects when required to do so and within yourclinical boundaries.
Person Specification Qualifications- Masters degree in pharmacy (MPharm) or equivalent
- Registration with General Pharmaceutical Council
- Specialist knowledge acquired through post- graduate diploma level or equivalent training/experience.
- Independent prescriber or working towards/intent of gaining independent prescribing qualification.
- Minimum of 2 years post- qualification experience.
- An appreciation of the nature of GPs and general practices.
- An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
- Excellent interpersonal, influencing and negotiating skills
- Excellent written and verbal communication skills
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences
- In depth therapeutic and clinical knowledge and understanding of the principles of evidence- based healthcare.
- Proficient in SystmOne clinical system.
- Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
- Able to obtain and analyse complex technical information
- Recognises priorities when problem solving and identifies deviations from the normal patters.
- Able to work under pressure and to short timescales on occasions.
- Produce timely and informative reports.
