Main duties of the job

Manage long term condition templates and recalls using clinical systemC

Chronicdisease management to include but not limited to Diabetes (specialist isdesirable) CVA, cardiovascular risk management, Heart failure, CKD, AF,Hypertension

Carry out cervical screening and sexual health services

Sexual health and contraception, possibly HRT reviews.

Perform wound care, dressings, removal of sutures and refer appropriately to wound clinic

Assist with treatment room procedures -minor ops and coil clinics

Vaccination and immunisation for adults, children and travel.

Monitor, interpret and act upon clinical observations and test results

Provide patient education and self-management support in accordance with National schedules

Undertake patient assessments, care planning and regular reviews in line with national and local guidelines

Ordering medical supplies

INR point of contact monitoring and advise

Health promotion and prevention care and advise, participate in screening programmes and health preventative initiatives

Identify safeguarding concerns and escalate appropriately

Maintain accurate, timely and confidential patient records

Contribute to Quality and outcomes (QOF) targets and audits

Participate in clinical audits, service development and quality improvement activities

We are a friendly, established, respected and forward-thinking medical practice situated to the market town of Ripley. We provide a first-class service for patients across a varied and developing practice area. The practice is a training practice and has a list size of 10,300 patients; our clinical team consists of 5 GP Partners, Salaried GPs, Practice Nurses, and a TNA We also have a compliment of PCN & ARRS staff including care coordinators, clinical pharmacists, pharmacy technicians and social prescribers, and are the current base for the ARCH Home Visiting Service, a PCN wide home visiting team.

Person Specification Skills
  • Substantial experience working as a General Practice Nurse in primary care setting
  • Experience working autonomously and managing own caseload
  • Experience managing patients with Type 1 and type 2 diabetes
  • Demonstrable experience of initiating, titrating and monitoring insulin therapy
  • Experience managing all long term conditions
  • Experience of QOF and chronic disease management templates
  • Knowledge of health promotion strategies
  • Experience using Emis

Qualifications
  • Registered nurse (Adult) with current NMC registration
  • Recognised qualification in diabetes care or equivalent demonstrable experience in diabetes management
  • Up to date knowledge of national and local diabetes guidelines
  • Experience managing patients with Type 1 and type 2 diabetes
  • Demonstrable experience of initiating, titrating and monitoring insulin therapy
  • Recognised post registration qualification
  • Recognised cervical screening qualification
  • Mentor / teaching qualification
