We have a vacancy for an experienced Practice Nurse who is passionate about delivering high quality nursing services and supporting patients living with a long-term condition.
This is a varied and interesting role and we welcome candidates with prior experience in primary care.
32 hours per week, over four days.
Main duties of the jobOffering care, support and education to our patient population including chronic disease treatment, screening and advice.
You will work with our team of Practice Nurses to deliver and support public health programmes, dressing and wound care, cervical cytology sampling, and participate in vaccination and immunisation programmes for both adults and children.

A minimum of two years post-registration experience as a Practice Nurse providing care to patients with long-term conditions is desirable but not essential.
About usBased at Truro Health Park, we look after over 18000 patients in Truro and the surrounding area.
You will be part of a dedicated team of GPs, paramedic practitioners, nurses, HCAs and physiotherapists.
Job responsibilities- You will undertake a range of nursingassessments and provide appropriate care / treatment in conjunction with GPs,other Practice Nurses or independently as appropriate for the need, accordingto Practice policies and protocols.

- Experience of working as a Practice Nurse.
- High level of clinical skills include cervical cytology, family planning/women's health, immunisation and vaccination, wound management.
- Well-developed communication skills in English, both written and verbal.
- Post registration experience in managing long-term conditions e.g diabetes and/or respiratory conditions in primary care.
- Nursing degree qualification with full registration of NMC.
- Qualification in long-term conditions.
- Knowledge of needs of patients with long-term conditions.
- Knowledge of health promotion strategies.
- Awareness of clinical governance issues in primary care.
- Knowledge of public health issues in the local area and wider health economy.
