We are seeking a dedicated and enthusiastic Practice Nurse to join our team at St Pauls Clinic. This role is pivotal in delivering comprehensive healthcare services, focusing particularly on childhood immunisations, chronic disease management, and wound care.
Main duties of the jobAdminister childhood immunisations and maintain accurate records.
Manage chronic diseases, providing regular monitoring and patient education.
Provide wound care and dressing management.
Provide patient education, support, and advice related to immunisation, chronic disease management, and other health promotion activities.
Manage patient attendance through proactive engagement, including telephone reminders and follow ups.
Collaborate with other healthcare professionals to deliver comprehensive patient care.
Maintain compliance with practice protocols, guidelines, and national clinical standards.

Job responsibilitiesThe Practice Nurse will provide high quality, evidence based nursing care within the primary care setting. They will focus on chronic disease management, childhood immunisations, wound care, and general treatment room duties. The role involves working collaboratively with the wider healthcare team to improve patient outcomes, reduce health inequalities, and support the practice's commitment to delivering patient centred care.
Key Responsibilities- Chronic Disease Management - Provide assessments, reviews, and ongoing management of patients with conditions such as diabetes, asthma, COPD, and hypertension.
- Childhood Immunisations - Administer vaccinations following national guidelines and maintain accurate records.
- Wound Care & Dressings - Provide treatment and follow up care for wounds, leg ulcers, and post operative dressings.
- Health Promotion & Prevention - Offer smoking cessation support, weight management, and lifestyle advice to patients.
- Women's Health - Conduct cervical screening, contraceptive advice, and menopause support, if trained.
- Treatment Room Duties - Assist with ECGs, phlebotomy, ear syringing, injections, and minor illness assessments.
- Medication Management - Support long term condition patients with medication adherence, reviews, and prescribing, if qualified.
- Work closely with GPs, pharmacists, healthcare assistants, and other clinicians to provide holistic patient care.
- Engage in multidisciplinary team meetings to discuss complex cases and contribute to practice development.
- Participate in continuous professional development (CPD) and maintain up to date clinical skills and knowledge.
- Assist in audits, quality improvement projects, and policy updates within the practice.
- Maintain accurate and confidential patient records using the clinical system EMIS.
- Ensure compliance with NHS Wales and Public Health Wales protocols, NICE guidelines, and infection control standards.
- HIW inspections and ensure the practice meets regulatory requirements.
- Strong interpersonal and communication skills.
- Proven ability to work independently and as part of a multidisciplinary team.
- Excellent organisational and record keeping skills.
- Familiarity with NHS protocols, clinical guidelines, and quality standards.
- Ability to engage proactively with hard to reach patient populations.
- Nursing qualification.
- Demonstrable experience in childhood immunisations, chronic disease management, and wound care.
- Experience in patient education and health promotion.
- Experience working in primary care or general practice.
- Experience in minor illness management and triage.
- Experience in women's health, including cervical screening and contraception advice.
- Experience working with multilingual or diverse patient populations.
- Experience in quality improvement initiatives or audits within a primary care setting.
- Experience working within socioeconomically disadvantaged communities.
