Physical Therapist- Home Health Visits in Senior Living

BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for full-time, part-time, or per diem opportunities providing home health visits at senior living facilities throughout York County, PA.

As a home care Physical Therapist, you will be an integral part of a multidisciplinary healthcare team that offers rehabilitative care and skilled nursing, enabling clients to receive necessary medical care while remaining at home.

Responsibilities include:

• Making home visits to clients within designated geographic areas.
• Performing diagnostic tests and measurements related to mobility, strength, balance, and safety.
• Implementing wound care interventions using evidence-based dressings and topical agents.
• Developing and adjusting individualized care plans, including manual therapy, gait training, and balance exercises.
• Assessing and revising care plans regularly and collaborating with nursing staff as needed.
• Educating clients and their families on rehabilitative activities to promote health, safety, and independence.
• Documenting observations, interventions, and evaluations accurately using electronic tablets.

Qualifications include:

• Current Physical Therapist (PT) license.
• Graduation from an accredited program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; experience with electronic medical records (EMR) preferred.
