Pharmacist

To be responsible for implementing an effective medicine management system within the practices, including cost-effective prescribing, working as an independent prescriber or willing to work towards independent prescribing status, providing advice and support for both patients and colleagues. In addition, the post-holder will also support the multi-disciplinary team, in line with the strategic objectives of the PCN.

Main duties of the job

The post holder will work within theirclinical competencies as part of a multi-disciplinary team to provide expertisein clinical medicines management, provide face to face structured medicationreviews, manage long term conditions, management of medicines on transfer ofcare and systems for safer prescribing, manage repeat prescriptionauthorisations and reauthorisation, acute prescription request, whileaddressing both the public health and social care needs of patients in the GPpractice(s) that make up the PCN.

About us

The Practice has a patient list size in excess of 13,000 patients. The practice is placed within a new large housing development which results in a younger population but still has a variety of health needs. The post holder will perform face toface medication review of patients with polypharmacy especially for olderpeople, people in residential care homes and those with multipleco-morbidities. The post holder will provide leadership on quality improvementand clinical audit and well as managing some aspects of the Quality andOutcomes Framework.

This role is pivotal to improving thequality of care and operational efficiencies so requires motivation and passionto deliver an excellent service within general practice.

The post holder will be supported todevelop their role to become a non-medical prescriber, if that qualification isnot already held.

You should ideally be working in community or a GP surgery as we would not prefer a newly qualified pharmacist due to being in a busy surgery

Person Specification Qualifications

• Currently registered member of the General Pharmaceutical Council (GPhC)
• Post-graduate clinical qualification, diploma level or higher
• Evidence of recent CPD and/or other professional development
• Prescribing Qualification the practice will support candidate to work towards an independent prescribing qualification
• At least 3 years post registration relevant experience
• Experience working in primary care with other healthcare professionals
• Experience in the care home sector
• Experience of working with CCGs/PCTs
• Experience of providing evaluated drug information

Experience

• A Prescribing Pharmacist would be an ideal candidate
• Previous experience in a GP surgery is desirable
