PERSONAL ADVISOR

Cardiff's Personal Adviser Team is a passionate and dedicated service supporting care leavers and unaccompanied young people; striving to achieve the best outcomes for our young people and support them into independence and adulthood.

The Personal Adviser Service provides young people leaving care with advice and guidance; assisting young people in their participation in pathway planning to ensure they properly reflect their individual strengths, aspirations and needs. The team also carries out appropriate assessments of individual's needs in collaboration with the Children Looked After Social Workers and other agencies as required.

About the job

The role of the Personal Advisor is to provide young people leaving care with advice and guidance; to assist young people in their participation in pathway planning to ensure that their pathway plans properly reflect their individual strengths, aspirations and needs. You will also need to make appropriate assessments of individual's needs in collaboration with the Children Looked After Social Workers and other agencies as required

The position involves a wide, varied and often changing range of tasks, so you would need to use your initiative, be able to work under pressure in a busy environment, and be committed to provide the best service possible to those requiring support.

What We Are Looking For From You

The successful candidate will have experience of assessments, person centre planning and care management skills working actively with all partners to identify and meet individual's needs.

The post holder is required to have good team working skills, the ability to produce innovative solutions and be forward thinking to help develop the service. Also the ability to liaise with other professionals, to be able to provide excellent outcomes for young people leaving care.
