Job Description
Personal Advisor - Leaving Care Team - £29.67ph
  • To provide the leaving care personal advisor role for all eligible, relevant, and former relevant young people as required by the Children (Leaving Care) Act 2000 and volume 3 of the Children Act 2010, including the production and review of the pathway plan and regular 2-monthly visits to care leavers.
  • To help young people contribute to the assessment of their needs prior to their transfer to the Leaving Care Team as prescribed by the Children (Leaving Care) Act 2000, and to engage in reviews and discussions of the pathway plan for 16/17-year-olds with CLAFH colleagues.
  • To work within the Redbridge Leaving Care policies and procedures.
  • To maintain appropriate statutory and administrative records, including the use of IT systems.
  • To maintain confidentiality and observe data protection guidelines.
  • To assist young people and their social workers when they reach 16 in their preparation for leaving care by providing advice and a range of group work sessions focused on developing independent living skills, careers, education opportunities, and identifying accommodation options.
  • To provide support to young people once they have reached 18 through individual and group sessions aimed at improving life skills, reducing social isolation, and enhancing peer support.
  • To maintain knowledge of accommodation providers and work with young people to ensure contact and security of licences or tenancies are prioritized, including completing application forms and ensuring smooth transitions to permanent council accommodation.
  • To develop and maintain positive links with agencies such as Connexions, career and training agencies, colleges, local employers, and the community to promote access to employment, training, education, and leisure activities.
  • To involve young people in planning, delivering, and evaluating services, and to encourage their involvement in the Children in Care council and other consultation systems in the borough.
  • To assess young people's financial needs and organize payments, benefits, or grants from educational or charitable funds, supporting them in managing their leaving care grants.
  • To participate in staff training and development programmes organized by the team or Department.
