About usThe Health Triangle PCN is a PCN has 3 practices looking after approximately 52,000 patients across Bracknell, Crowthorne, Owlsmoor and Sandhurst. As well as delivering excellent primary care, we are fortunate in having clinicians with expertise in cardiology, haematology, palliative care, immunology, dermatology, minor surgery, joint injections, LARCs and frailty. There is a strong ethos on supporting one another, as well as on education. We offer protected educational and clinical meetings several times a month and mentor ST and FY doctors, medical students, nursing students and pharmacy students. We are also highly active within our ICB and Federation.
Job responsibilitiesWe are a friendly and forward-thinking PCN spanning over Bracknell, Crowthorneand Sandhurst. With a large GP team and extensive clinical team, we aim todeliver excellence and innovation in primary care.
There is a strong ethos on education as well as on supporting oneanother, with each salaried having a GP Partner as a mentor. We provide educationaland clinical meetings several times a month (including one full afternoon permonth practice closure for protected teaching session) and train GP STs and FYdoctors, medical students, nursing students and physician associatestudents.
We are highly active within our ICS and Federation, with a number of clinicallead positions being held within the ICB. The PCN is highly innovative having delivereda number of exciting projects already, including a Cancer Quality Care Project anda Dermatology Drive-Through Service which has received national recognition forour patients.
