Job Description
Gloucester Inner City Primary Care Network is seeking adedicated and experienced Specialist Diabetes Lead Nurse to support thedelivery of the PCNs Diabetes Enhanced Service. This is a unique opportunityto be part of an innovative, integrated approach to diabetes care, workingclosely with a multidisciplinary team including Diabetes Specialist GP,Practice Nurses, Pharmacists, Health Care Assistants, and secondary carepartners.
The role will primarily focus on providing clinicaldelivery, leadership and management of red-risk type 2 diabetic patients,improving outcomes in line with national treatment targets, and reducing riskstratification levels. The postholder will be instrumental in deliveringproactive, patient-centred diabetes care with a strong emphasis on earlyintervention, medication optimisation and initiation, lifestyle support, andcontinuity of care. The role requires support and mentoring of Diabetespractice nurses across the PCN.
Working Hours: 22.5 hours per week (3 days) 1 day work from home,2 days on site rotation across surgery sites across the month proportionatebased on patient list size.

To foster an effective, quality driven PCN diabetesservice.
To support the development of GloucesterInner City PCNs diabetes services for registered patients of all practiceswithin the PCN.
Work in compliance with CQC, statutory and NHS requirements applicableto the PCN diabetes service
To provide a high standard of specialist diabetes nursing care forpatients, as well as providing diabetes nursing assistance to the doctors andother members of the primary healthcare team.The duties will include all tasks normally undertaken by an experiencedRGN and any additional roles agreed between the nurse and practice managementteam as appropriate, having regard to the nurses training and competencies.

Job responsibilities Duties Specific to Role
Leadership
PCN diabetesnurse team leadership and supervision.Lead, support and manage other members of the PCN diabetes team,including trainees and HCAs.
Delivera dedicated diabetes specialist Lead Nurse clinic targeting patientsstratified as Red Risk under the PCNs diabetes risk model.
Reviewcomplex cases and provide specialist input into the diagnosis, monitoring,and treatment of patients with Type 2 diabetes.
Initiateand optimise pharmacological treatment including insulin, GLP-1 receptoragonists, SGLT2 inhibitors, and other oral hypoglycaemic agents, inaccordance with NICE and local guidelines.
Managefollow-up pathways including blood testing, medication titration, and patienteducation.
Liaisewith Diabetes Practices Nurses to determine clinic lists and monitor progressacross the cohort.
Activelyparticipate in monthly MDTs including integrated neighbourhood working andencourage the PCN to engage in a wider-system approach to Type 2 Diabetescare.
Completethe PCN-approved diabetes template on SystmOne for every patient seen. Ensureall consultations, interventions, and follow-up actions are clearly recorded.
Codethe initiation of medications and all diabetes-related activity accuratelyusing appropriate clinical codes to support safe prescribing, data quality,and audit readiness.
Provide diabetesclinical care to patients registered with PCN practices, within yourspecialist skills and competence. Primary care specialist diabetes nurseduties including (but not limited to) face to face consultations, videoconsultations, telephone consultations, e-consultations and queries. Clinical administration, and dealing withqueries, paperwork and correspondence in a timely fashion.
Provision ofcare for acute and long-term diabetes conditions and contributing to thedelivery of QOF and enhanced services.
Prescribing in accordancewith locally agreed or national guidelines.
As a qualified non-medical prescriber(NMP)/independent prescriber (IP), prescribing will be undertaken within thescope of practice of the prescriber and in accordance with their professionalcode of conduct as set out by their own regulatory bodies and organisationalNMP policy
Provision ofpreventative care, including health promotion advice, vaccinations andscreening
Support ofpatients in the developing self-management strategies and in fosteringpatient engagement with the PCN Practices.
Working alwayswithin your professional competencies, you will make yourself available toundertake a variety of duties including surgery consultations andemergencies; telephone consultations and queries; triaging; and dealing withqueries, patient test results, patient medical reports and examinations; andNHS/private paperwork and correspondence.
Training andpromotion
Ensure skills ofPCN diabetes nurse team areappropriately and effectively used to deliver care. Support the PCN diabetes team in developingtheir skills and expertise
To be aware of existing skills within the PCN and highlighttraining needs specific to diabetes services, along with an awareness oftraining opportunities, locally and nationally.
Provide diabetestraining and support to nurses within Gloucester Inner City PCN Practices.
Work acrosstheir PCN to promote awareness and engagement with the best practices for diabetiscare, ensuring a consistently high standard across the PCN.
Attend allpractice PLTs at least once a year to provide diabetes training and update onthe PCN diabetes service.
Attend PCN BoardMeeting annually to provide update on PCN diabetes service.
Plan, develop,implement, and evaluate new diabetes health promotion and long-term conditioncare.
Advice andsupport
To be apoint of contact for practice staff members regarding delivery of diabetescare.
Provideprofessional advice to practices on diabetes developments, recommendingchanges to working practices in line with evidence-based knowledge. Lead andmonitor the development and implementation of diabetes policies, standardsand guidelines across the PCN diabetes team and practice nursing teams.
Operational
Ensure aneffective nurse and appointment system, in collaboration with PCN Business Managerand PCN Project Manager.
In collaborationwith the PCN Business Manager and the Recruitment Team, manage therecruitment, onboarding, induction and ongoing employment of the PCN diabetesteam including nurses, healthcare assistants and health and wellbeingcoaches.
Monitor andadvise on the effective use of physical and financial resources by the PCN diabetesteam, in partnership with the PCN Business Manager.
Ensure a robustsystem for PCN diabetes nurse appraisal and revalidation
Cover for other diabetesnurses will be provided by reciprocal sharing of correspondence, results andother administration.
Quality andClinical Governance
Working with theClinical Directors and Lead Diabetes GP, develop and monitor key qualitymarkers, thereby promoting clinical effectiveness.
Enable the PCN diabetesservice to apply and improve clinical governance and risk management systems.
Operate in line with national and local diabetesguidelines and PCN protocols.
Ensure that you are familiar with CQC, NHSand NICE requirements relevant to the diabetes service and support the teamin understanding them. Areasof non-compliance must immediately be escalated to the PCN ClinicalDirectors.
Ensure that you are familiar with the Health and SocialCare Act 2008 Code of practice on the prevention and control of infectionsand related guidance and that the diabetesteam is compliant with them. Areas of non-compliance must immediately beescalated to the PCN Clinical Directors.
To meet with PCNClinical Directors, quarterly as a minimum, to ensure appropriate clinicalgovernance and oversight of the service.
Making professional, autonomous decisions in relation to presentingproblems, whether self-referred or referred from other health care workerswithin the organisation.
Recordingclear and contemporaneous I.T. based consultation notes to agreed standards;
Collectingdata for audit purposes as per Practice protocols.
